First Exam:

When you arrive, please sign in at the front desk. We will give you forms to fill out pertaining to you and your pet. To help save time at your first visit you may print and fill out our new client forms at home and bring with you. Please refer to our On-Line Forms section in our website.  Please bring any pertinent information, copies of any laboratory results, medical information, and medications given to you by your veterinarian pertaining to the eye issue. It is also helpful if you could double-check with your veterinarian's office to ensure that they have faxed or mailed us copies of the medical record pertaining to the eye issue and the Patient Referral Information Sheet. Expect your first visit to be about an hours time for a comprehensive eye exam.

Typed discharge instructions will be faxed or mailed to your referring veterinarian after each visit, so that we may provide the best quality care for you and your pet through excellent communication with the specialist.  Please provide relevent information regarding your pet's medical history in addition to current or past therapy for the eye.  Please provide your veterinarian's name and address when completing our New Patient Information sheet.  Remember, excellent communication between the specialist and your veterinarian will ensure the very best care for your pet.  Thank your veterinarian for entrusting us with your pet's eye care needs.  Please return to your veterinarian for all other health care concerns.  Our specialty services are a great compliment to your family veterinarian in providing the highest quality care possible.
