To help save time at your first visit you may print and fill out our new patient and eye history form at home and bring with you.
Prescription Refill Request Form
For your convenience please call in refill requests 24 hours in advance before running out of medication. This way your prescription can be ready for pick up to avoid unnecessary waiting. Please call our office at (562) 943-3728 or use our new online request form. As an added convenience, we can also ship refill prescriptions to your home or office. Additional forms are required to be filled out and additional fees apply.
